Unknown. Maybe related to French faire gaffe (“pay attention”), or German gaffen (“to stare”). Possibly related to the Andalusian Arab scholar of the 12th century Muhammad ibn Aslam Al-Ghafiqi, an ophthalmologist. Eyeglasses “gafas” were named after him.
